This card identifies the holder as a member of the ALTRAMED® health products Loyalty program. It is not a credit or debit card. ALTRAMED® Health Products reserves the right, at any time, to amend or terminate the benefits of the card holder in accordance with such terms and conditions, which may also be amended from time to time. For every dollar spent you can receive one point towards rewards. Rewards may be redeemed with a purchase only, some restrictions may apply. Provide or enter your ALTRAMED® Rewards card number at the time of transaction to earn or redeem points. When you obtain your ALTRAMED® Rewards Membership card, 250 points will be added to your balance if the card it is used within the first 90 days.
